In order to find claims sent to America's Choice Healthplans®, please enter your tax id number, the insured's ID number, the patient's birth date, and the date of service that appear on the claim. Please note, if searching for a claim with multiple dates of service, enter the earliest date of service listed on the claim. Once you have successfully located one claim for the member, you will be able to view all of the claims that you have sent to America's Choice for this member.
